Home Shield Norfolk
Home Shield Norfolk is a cross-referral service, for vulnerable and older people and their carers. The service is supported and coordinated through a group of partner agencies whose aim is to find ways to enable people to stay safe, healthy and happy in their own homes. The kinds of things these partner agencies help with are:
Health and well-being
* Reducing hospital admissions
* Helping clients to live independently in their own homes
* Identifying problems before they become acute
* Referring people with unmet needs to the right support services
Income
* Checking that people are receiving all the income and benefits to which they are entitled
Daily living
* Ensuring the client gets regular hot meals and help to access grocery shopping
* Advising on how to keep their house warm and to have access to hot water and toilet facilities
* Helping the client enjoy regular social contact
Security and safety
* Finding ways to help the client to be able to secure their house against unwelcome visitors
* Making sure that clients do not fall foul of scams or other forms of “rogue” trading
* Ensuring the client has a working smoke alarm
* Checking for any fire or trip hazards in the client’s home
If a front line officer, meeting a client in the course of their ordinary business, finds that that client has a need which the officer cannot address but which could be fulfilled by another agencies, the officer will refer it, through Home Shield, to the other agencies. The officer will briefly explain Home Shield and complete the referral form which must be signed by the client to show that they have agreed to their details being referred to partner agencies.
The officer then faxes (or posts) the referral form to the Home Shield Co-Ordinator (within two working days) who will pass it on to the relevant partner agencies. The partner agencies have a responsibility to act upon the referral within ten working days, wherever possible. When the referral has been dealt with, the partner agencies will feed that information back to the Home Shield co-ordinator.

How the information we hold about you will be used – Information about you will be kept by the agency who has visited you, Home Shield ( Norfolk County mCouncil) and any agency that Home Shield will refer you to. This information will be stored to ensure that you receive the most appropriate service in a timely manner. This information will be kept in accordance with legislation.
