
Medical Discharges
If you are medically discharged from the Armed Forces, the SPVA will, in some circumstances, automatically consider any illness/injury that contributed to your medical discharge, without the need to submit a claim form. For more information, see our Medical Discharges section
Claiming a War Disablement Pension
In all other circumstances, claims for compensation
under the Scheme must be made on a AFCS/WPS01
claim form signed and dated by the claimant, or by someone
authorised to do so on their behalf.
Application for a review
If you already have entitlement to a War Disablement Pension but your condition changes, you can ask us to review the assessment of your disablement. You can do this by completing WPA 0012 Application for a Review
Claim for a further condition
If you already have entitlement to a War Disablement Pension and want to claim for an additional medical condition, you should complete WPA 0002 Further Condition Claim Form
If you want a representative to help you
If you wish to nominate a representative to help you with your claim, we need your permission to send details about your claim to them. To give us this permission, please complete the reply part of this form. WPA 0001B
Personal Injuries (Civilians) Scheme
If you were a civil defence volunteer (CDV) disabled through serving as a CDV or a civilian disabled as a result of enemy action in the 1939 to 1945 war, you should complete WPA 0001C War Disablement Pension under the Personal Injuries (Civilians) Scheme Claim Form
Mercantile Marine claims
If you were a merchant seaman, a member of the naval auxiliary services or a coastguard, and you were disabled because of an injury you received or a disease suffered because of conditions during a war or because you were a prisoner-of war, you should complete WPA 0001M Mercantile Marine Claim Form
Polish Forces under British Command
If you were a member of the Polish Forces under British
Command who served in the 1939-1945 war, or in the Polish
Resettlement Forces and you were injured or disabled
through this service, you should complete form AFCS/WPS01.
Child Allowance claims
You may be entitled to this allowance for children under the age of 16 if their parent:
- Died due to service in HM Forces
- Died because they were a civilian or a civil defence worker who was disabled due to the 1939 – 1945 war
- Died because they were a merchant seaman, a member of the Naval Auxiliary Services or a Coastguard, who was disabled because of an injury or disease they got during a war, or due to the effects of detention by the enemy
- Was getting, or could have got, a War Pensions Constant Attendance Allowance when they died.
Children over the age of 16 may be entitled to an allowance if they are in full time education at a school, college or university. To make a claim, complete form WPA0011A Child Allowance Claim Form
