AFCS01
Claim Form
This form should be completed if you want to make a
claim under the Armed Forces Compensation Scheme. You must be serving or
have served in HM Forces (including Reserve Forces and Brigade of Gurkhas)
and your illness or injury was caused by your service on or after 6 April
2005. If you are no longer serving and your illness or injury was caused
by your service before 6th April 2005, you may be considered under the
War Pensions Scheme.
AFCS02 Dependants
This form should be completed if you want to make a
claim under the Armed Forces Compensation Scheme and you are the dependant
(or the Guardian or legal representative of a dependent child) of a former
service person who died after leaving service on or after 6th April 2005.
Claims must be made within 1 year
of the death.

details Form

This form should be completed to confirm Dependant’s details
and to make a claim for children under the Armed Forces Pension Scheme
(AFPS) and/or the Armed Forces Compensation Scheme (AFCS), where death
occurred in service on or after the 6th April 2005.
